How to make a claim

To make a claim you need to complete an emergency claim form and return it to Denplan. It is a condition of the benefit being paid that any claim should be received within 60 days of the incident.

Don’t worry if you or the dentist do not have a claim form to hand at the time of treatment; simply ask them to write details of the emergency on the surgery’s headed paper and sign it. Attach these details to a claim form and send them to us.

Claim forms can be obtained from your dentist or from Denplan on freephone 0800 0850960. Lines are open 08:30 to 17:30 Monday to Thursday, and 08:30 to 16:30 on Fridays. Alternatively, you can download a claim form by using the links below.

Before submitting your claim you should check the scale of benefits, as payments will only be reimbursed up to policy limits. Please click on the relevant benefit for more details.
