Making a claim
If you have received eligible treatment when away from your registered
Denplan dentist as a result of injury or an emergency situation, the dentist
will either ask you to sign an insurance
claim form, so they can claim back the cost from us, or ask you to pay
at the time of treatment.
If you are required to pay for treatment at the time of your visit, please
ensure you obtain a receipt for your payment and then submit your claim,
complete with the receipt, to Denplan for reimbursement.

To make a claim you need to
complete an insurance
claim form and return it to Denplan. Claims should be
received by us within 60 days of the incident. |

| Step 2 |
If you do not have an insurance claim form to hand
at the time of treatment, it is very important to secure a receipt for
the amount you pay.
Also ask for written documentation of the treatment you receive, set out
on the dentists headed paper and including their signature. |

| Step 3 |
In certain circumstances, Denplan must authorise the
treating dentist and planned treatment before it is carried out.

policy document for details. If in doubt, call our Insurance team. |

| Step 4 |
Treatment costs will be reimbursed up to the benefit
limits specified in your Supplementary Insurance policy document.
Any charge which exceeds the specified benefit limit must be paid by you
directly to the treating dentist. |

Call the
Insurance team at any stage of your claim for guidance and advice. |
