Changing your dentist

If you are moving house, going to college or if there is a change in your circumstances, you may need to change your dentist.

Your Denplan contract is between you and your dentist, therefore please follow the simple steps below to ensure a smooth transition to another dentist:

Steps Description
Step 1 Arrange a leaving appointment with your current dentist / practice.
This will give your dentist the chance to finish off any outstanding treatment that you may need.
Step 2 Ask your dentist to complete your Patient Leaving form.
You will need to give this to your new dentist, as it will give them valuable information about your past dental history.
Step 3 Write to or telephone us, giving at least 21 days notice to cancel.
You need to end your contract with your present dentist. Notice will expire on the last day of the month and your Supplementary Insurance cover will also stop at that time.
Step 4 Re-register with a new Denplan dentist.
To help you find a dentist, please use our online 'Find a Dentist' service. Please remember to give your new dentist your completed Patient Leaving form.
Step 5 Check if there will be a fee for your new Denplan Assessment.
Check this before booking your first appointment with your new dentist as this can vary from practice to practice.
Step 6 Complete a new Registration form and Denplan Contract at your first appointment with your new dentist.
If you re-register with a Denplan dentist within six months of ending your previous registration, you can re-register free of charge.
Step 7 Check your new monthly fee.
As Denplan dentists set their own fees, your monthly fee may have changed.
Step 8 Check that your dentist is happy with your treatment needs.
Remember that clinical opinion between dentists can and do vary. Your new dentist may have a different opinion of your treatment needs, and there may be a difference in what is included in your contract.
