Making a claim

If you have received eligible dental treatment when away from your registered Denplan dentist, as a result of a dental injury or dental emergency, the dentist will either ask you to sign an Insurance claim form so they can claim the cost back from us, or ask you to pay at the time of treatment, and you claim from us.

If you are required to pay for treatment at the time of your visit, please ensure you get a receipt for your payment and then submit your claim, complete with the receipt, to us for reimbursement.

Do you need authorisation prior to treatment?

In certain circumstances, we must authorise the treating dentist to provide the planned treatment before it is carried out.

Check the Supplementary Insurance Policy summary for details. If in doubt, contact our Insurance team.

What if I don’t have a claim form?

If you do not have an Insurance claim form  to hand at the time of treatment, it’s very important to obtain a receipt for the amount you pay. Ask the dentist for written documentation of the treatment you receive, on the dentist's headed paper, including their signature.

Submitting your claim

Step 1

To make a claim you need to complete an Insurance claim form and return it, with proof of treatment, to us. Claims should be received by us within 60 days (wherever reasonably possible) of the completion of your dental treatment.

Step 2

Your treatment costs will be reimbursed up to the benefit limits specified in the Supplementary Insurance Policy document.

Any charges which exceed the specified benefit limit must be paid by you directly to the treating dentist.
