Step 01 At the practitioner
Obtain proof and details of treatment from your dentist, doctor or physiotherapist
You must obtain fully itemised receipts with a breakdown of treatment costs and proof of payment.
Step 02 Complete your claim form
Complete a claim form and attach proof of treatment and proof of payment. Please use one claim form for each course of treatment, and a new form for each child.
