Claiming online
If you've had emergency dental treatment in the UK, you can make a claim online. You'll need to have the following to hand:
Your details, including your membership number
Details of the dentist who treated you
Details of the appointment and the treatment you received
A scan or picture of the receipt from the treatment, in one of these file formats: BMP, JPEG/JPG, ODT, PDF, PNG or TIF

Claiming by post
To claim by post, first download the correct claim form from the list below.
Fill in the details and return it to Denplan with proof of the dental treatment you've had.

Claiming by email
Claiming via email is much like claiming by post, to get started you'll need to download the correct claim form from the list below.
Once you have the form, fill in the details and return it to [email protected] with proof of the dental treatment you've had.

Things to consider
All claims should be sent within 60 days of your treatment
If your claim is successful, payment will be sent to the account under your patient registration number. So, if someone else pays for your Denplan plan, their account will receive the payment.
Treatment costs will be refunded up to the benefit limits outlined in your policy document. Any changes outside the benefit limit are to be paid by you directly to the treating dentist.
