Denplan Champion incentive terms and conditions

This “Incentive” is a reward scheme which encourages Denplan Champions to sign up new patients; in return for each new patient signed up via the online patient application form, the Denplan Champion will be able to redeem the value of a £1 Amazon voucher reward (the “Reward”) or such other equivalent reward scheme as the Promoter may decide at its discretion.

The “Denplan Champion” is an employee, officer, staff member, other worker, agent, locum and/or consultant, who signs up to be a Denplan Champion and is engaged in the performance of dental services at a dental practice contracted with the Promoter.

Terms and Conditions

Anyone who is a Denplan Champion may participate in the Incentive.
The Incentive is open for Denplan Champions to participate in until 31/12/27 (the “Reward Period”). Any new patients registered by Denplan Champions outside of the Reward Period will not result in a Reward.
Any attempts to register the same patient more than once will not result in additional Rewards being accumulated.
If the Denplan Champion leaves the dental practice contracted with the Promoter, all non-redeemed vouchers will be irredeemable and non-transferable.
The Reward is non-exchangeable, and there is no cash alternative.
Entry into the Incentive and sign up to be a Denplan Champion is deemed as acceptance of the rules.
The Promoter reserves the right to replace the Reward with similar alternatives of equal or higher value if circumstances beyond the Promoter's control makes it necessary to do so.
Rewards are calculated by the Promoter monthly. An email will be sent to each Denplan Champion after the calendar month has ended, setting out the Rewards accumulated for the preceding calendar month, with an e-code to redeem the voucher Rewards. Details of how long the e-code will be valid for will be shared in the monthly email referred to in clause 8. The e-code will not be replaced for any reason. E-codes are subject to Amazon UK Services Ltd terms and conditions, as amended from time to time, which can be found on their website: www.amazon.co.uk
The decision of the Promoter regarding any aspect of the Incentive is final and no correspondence will be entered into with Denplan Champions.
In the event these terms and conditions are breached, the Promoter reserves the right to remove Denplan Champions from participating in the Incentive.
The Promoter will not be liable:
(i) For any new patients that sign up via the online patient application form but are not properly registered as towards the Reward due to technical or connectivity issues, or other problems;
(ii) For any delay in you receiving the Reward; or
(iii) In the event that the law or any regulatory guidance prevents the Promoter from continuing the Incentive.
The Reward is subject to the terms and conditions of Amazon UK Services Ltd (Amazon or such other equivalent reward scheme as the Promoter may decide at its’ discretion), as amended from time to time. To the extent permitted by law, the Promoter accepts no responsibility for the Reward.
The Promoter reserves the right to void, cancel, suspend, or amend the Incentive if it becomes necessary to do so without prior notice. Any amendments to the terms and conditions of the Incentive or information regarding its withdrawal will be published on the Promoter’s website.
You are not eligible to participate in the Incentive if you are an employee of an entity within the Simplyhealth Group.

Denplan will pay all Income Tax at basic rate for the recipient that is attributable to the value of the voucher award. Any recipient who is liable to pay Higher Rate or Additional Rate tax will be responsible for declaring and paying any additional tax liabilities directly to HM Revenue & Customs through their tax return.
The Incentive is governed by the laws of England and the courts of England and Wales shall have exclusive jurisdiction to settle any dispute or claim (including noncontractual disputes or claims) arising out of or in connection with these terms and conditions on their subject matter or formation.

The “Promoter” is Denplan Limited. Denplan is a trading name of Denplan Limited. Denplan Limited is incorporated in England and Wales, company no. 01981238.
